Primary Adrenal Insufficiency

  • Def:
    • pathology level of the adrenal glands
    • ACTH level will be increased due to lack of suppression
  • Management:
    • glucocorticoid and fludrocortisone therapy

Secondary Adrenal Insufficiency

  • Def:
    • MC pituitary disease
    • pituitary infarction
    • large pituitary tumor
  • Labs:
    • decreased plasma cortisol levels
    • low plasm ACTH levels
    • hypoglycemia
  • Management:
    • only glucocorticoid replacement

Tertiary Adrenal Insufficiency

  • Def:
    • suppression of the hypothalamic-pituitary-adrenal axis by chronic glucocorticoid administration and sudden withdrawal of glucocorticoid
  • Management:
    • only glucocorticoid replacement
